I do not accept any insurance . I will however be happy to provide a superbill at the end of each month if so requested for any out of network reimbursement you might be entitled to. It is advised that you speak to your provider before services start to understand better your coverage for out of network services. A superbill will require a diagnosis if applicable and will be a part of your health records.
I am on a few EAP ( Employee assisted Program) panels and will be happy to provide you with the details during our initial phone consult. You can also inquire who your EAP provider is before we connect.
I do not provide sliding scale rates.
I accept cash and check as forms of payment.
If you are unable to attend a session, please make sure you cancel at least 24 hours beforehand. Otherwise, you may be charged for the full rate of the session.
